If you are a Primary Care Physician (“PCP”), a physician, hospital, or ancillary provider and wish to offer services through one of our contracted IPAs or Medical Groups, please contact our Provider Services and Contracting Department. Please make your request in writing, specifying the Medical Group/IPA you wish to join. We will send you a Pre-Contractual/Minimum Physician Standard Attestation form to be completed. Upon receipt of the completed form, your request will be reviewed and presented to the appropriate Medical Group(s) / IPA(s) Network Expansion Committee and/or Board of Directors. Upon Board approval, a complete credential application and agreement will be sent to you for review and completion. Please allow between 4 to 8 weeks in completing this process.

Our managed Medical Groups/ IPAs are always looking for opportunities to expand their provider panels to include high caliber providers interested in providing quality services to our members.
